Sick Notes

You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days.

If you have been sick for more than 4 days in a row, but less than 7, you can self-certify using a SC2 self-certificate form. This can be found on the HMRC website Follow this link to the online Statutory Sick Pay form or your employer might be able to provide you with one.

We are asking that all requests for sick notes are sent via Patchs, our online consultation programme.  Usually, you will not need an appointment with a GP or Nurse to discuss a sick note but should it be necessary, the Clinician will contact the patient to find out any missing information or ask further questions about the sick note request. By submitting sick note requests via Patchs, this will free up some GP and Nurse appointments for those who urgently might need them for their clinical condition. Please note that sick notes cannot be dated in advance and please allow clinicians 48 hours in order to issue them.
